Life insurance is a benefit offered to Full and Part time employees by the CLIU. It is important that you and your beneficiary are aware of your coverage.
If you are eligible for life insurance, it is important for you to maintain an up to date beneficiary designation with the CLIU. If we do not have a beneficiary designation on file, your life insurance benefit may not be given to the person you wish to receive the monies. You may update your beneficiary information at any time by printing the Beneficiary Designation Form,
completing the information requested, and submitting it to the Human Resources Department. We recommend you keep a copy of the form for your records.
Life insurance certificates provide you with the terms and conditions of the policy the IU has purchased on your behalf. Certificates may be found in the Plan Designs and Booklets section of the Benefit Blueprints.
