| Transportation Request to a Non IU Classroom |

Transportation has set forth a new procedure for transporting Non IU Students. Members now have to download and submit the entry/withdrawl form by entering in their username and password below. Please complete the entire form and send it to Karen Papay at papayk@CLIU.org. The entire form must be completed before sending to the CLIU. If any changes, a new form must be sent for process.
